A "control chart" is a picture of the process as it occurs.  I think of it as foot prints in the snow.  There are many styles and methods for control charting.
 

Here's how one basic type is built:

 

Each time a measurement is made, a new mark is placed on the control chart.

 

We add lines that connect the measurements so we can see patterns:
